Wiki Is patient resource navigation billable?

Hi, I was trying to find out if there is a code that can be used for patient resource navigation? For example, emergency medicine trying to get patients set up with a PCP or specialist and doing care coordination work? If so, would the staff need certain credentials in order to make it billable? Any help is appreciated.
