Wiki Is a UA payable with a Medical Diagnosis

We are located in the state of Michigan. We have been trying to determine which payor's if any consider a UA with a medical diagnosis when billed with a E/M or a Consult. From what we have seen they are inclusive to the E/M service. Can someone shead some light on this matter.

This is on an individual payer basis. Some of the larger commercial insurance carriers began including the dipstick UA in the fee for the E/M service provided on the same date of service.
