I have a physician that started an H & P for a pediatric pt on 10/8 (she documented 10/8 on the first page) but when she was finished she signed and dated it 10/9 at 12:15am. Do I count 10/8 as the Initial IP care visit or 10/9? The pt was discharged on 10/9. Your help would be appreciated. Thank you.

Most likely your physician finished the encounter with the patient prior to midnight on 10/8, and also started her documentation before midnight 10/8.

I would say use 10/8 as the initial IP visit, as that is the date when your physician had the first encounter with the patient. Since the patient was d/c'd on 10/9, 99238 or 99239 would be reported on that date (provided the dischage requirements were met).
