When do you choose 66985 (insertion of IOL(secondary implant) not associated with concurrent cataract removal).

66985 does it mean patient does not have cataract?

I am doing some exercise with Dx 371.23(keratopathy) ,glauaucoma (365.10)
and the procedures are Keratoplasty (65750)
AND
Posterior chamber IOL scleral implant (CPT Code ????)

When to use 66985 vs picking one from 66982-66984

I would chose this procedure only when all that is happening today is insertion of the IOL; the cataract removal was done previously, before today.

If the physician does the IOL insertion concurrently with (in the same session) the cataract removal, you would do not use 66985. In this situation, depending on what the details in the OR, you would use one of these:

66983 Intracapsular cataract extractin wth insertion of intraocular lens prosethesis (1-stage procedure)

66984 Extracapsular cataract removal with insertion of intraocular lens prosthesis (1-stage procedure)

or

66982 Extracapsular cataract removal with insertion of intraocular lens prosthesis (1-stage procedure), manual or mechanical technique (eg, irrigation ans aspiration or phacoemulsification), complex, requiring devices or techniques not generally used in routine cataract surgery (eg. iris expansion device, suture support for intraocular lens, or primary posterior capsulorrhexis) or performed on patients in the amblyogenic developmental stage

I told that most (98% of) cataract removals are extracapsular, so, most likely 66984 or 66985 if it meets the complexity requirements.
