could someone help me code this report please.
Following diagnostic cerebral angiography of the right internal carotid artery the diagnostic cath. was exchanged over for a 6f cordis. Through the Envoy a SL-10 microcatheter was positioned into the right m2 segment. The microcatheter was then exchanged over to a transend 300 microwire for a 4.5x30mm neuroform 3 stent. The stent was deployed across the neck of the aneurysm. Post deployment IADSA images were obtained to assess vessel patency and stent positioin. The microwire was repositioned into the aneurysm, however the microcatheter would not track into the aneurysm. Multiple attempts were made to gain access to the aneurysm. During the process, the microwire became snared in the distal aspect of the stent, displacing and deforming the distal aspect of the stent further into the petrous segment of the carotid artery. The procedure was aborted at this time to allow the stent to heal into the artery. The guide catheter was then removed under direct fluoroscopic visualization.
