Wiki Intertrochanteric revision help????

We did a "treatment of intertrochanteric fx with im implant" in Decemeber. Apparently the screwws started to "back out" so the doctor went back in with a screw driver and pushed the screws back into place. How would I code this? He wants to use 27245 which of course, I think is wrong since it is a "revision" so to speak. HELP! Thanks.

I would use that and use the reduced services modifer as it is not as much work as the original but still is the same procedure just less work involved as it had all been done before.
