Hello, if anyone is available to help me with a doubt I am having it will be very helpful.

If a Physician documented: depression/anxiety, what is the correct coding method that I can use for this medical assessment?

Just stating depression/anxiety isn't enough for an evaluative statement. Was it controlled or stable? You couldn't code for either diagnosis without supporting documentation.

I gather that the question was more about the significance of the symbol and how that would affect coding than about how this specific statement should be coded. If so, I'm not aware that the "/" symbol has any specific meaning that affects diagnosis coding. I would interpret it to be the equivalent of 'and', or 'or', and would code accordingly, but to say any more than that would require a query to the physician to get clarification or additional detail.
