I work in a teaching hospital and we have residents who believe an intercostalbrachial block doesn't require a separate procedure note as it's a supplemental block to certain types of brachial plexus blocks and it's not payable separately. I saw a previous thread about this block asking how to code it but for those of you who see this block being performed, are you seeing a separately procedure note as well?

If that block is being used as the primary anesthesia, then you would code out the procedure & crosswalk it to the CPT anesthesia code. It should, however, be documented as to how the anesthesia was delivered.

If that block is provided for post-op pain management, they definitely need to provide the documentation for the block, the time it was administered, & the method along with the ultrasound pix. For post-op pain, the block is billed out separately with a 59 modifier.

Either way, the block needs to be documented. Hope this helps. :)
