Wiki Insurance company splitting my claims?

I am billing for a family practice and noticed that Medicare has been splitting my claims. Im not sure why. Generally, I bill 99214 as a visit then add the procedures done. Some EOBs are coming back denied for more information, and those include just the procedures, not the Office visit.

I ran everything through a claim scrubber and all the codes and modifiers were correct.

Anyone have an idea?

What procedures are you billing along with the office visit? Some codes require a procedure comment to describe the service rendered. I have noticed that our MAC splits the CPTs out that can be processed to expedite payment processing.
