Does anyone know the revenue code for billing the CPT code (90834) on a UB04? Would it happen to be 0780 or 0789?

The facility is Part A Medicare Provider and all their claims were rejected. They are located in FL. Would we have to call Medicare in FL to find out how to reprocess these claims. If anyone has a phone number to share that would be very helpful.

I have looked all over the internet and was not able to find how to bill this correctly.

Was the facility the originating site? if so you are using the incorrect code.

Yes it is a facility and they are only credentialed in Part A. They are the originating site.

to bill for the originating site you must meet the requirement of being located in
* a county outside of a Metropolitan Statistical shortage area
OR
* A rural Health Professional Shortage Area located in a rural census tract.
Bill the originating Site Facility fee code of Q3014.

They do qualify for this benefit. They checked with Medicare.

you can send to me but if you are billing with the 90834 then that is the incorrect for the originating provider.. that is what the Q3014 is for.
