I am reviewing inpatient charts that have a debridement procedure code (for the RAC).
I came across one that was coded with Exc Debridement (86.22) as well as revision of stump (84.3). Please help me understand what is involved with an amputation revision. My understanding was that the circumference of the bone had to be cut away.
Here is what was documented on the OR record:
.....was take to the OR and underwent debridement of the medial left above-the-knee amputation. A fair amount of dead muscle was removed, as well as the large eschar. Tendons were debrided as well. Hemostasis was controlled..
HIM removed the 86.22 debridement but kept the 84.3. Any advise or information is appreciated.
