It is my understanding if someone comes in for a clinic visit and the Dr. decides to do a set of three injections, we can charge an E&M visit for the 1st injection but the next two visits we can only charge for the injection. So, what if a patient comes in and an injection is discussed but no injection is given and an appointement is set for that patient to come back in two weeks for the injection, can you charge for another E&M visit and the injection? Thanks.

No once the medical necessity for the service is established (the E&M). You cannot charge it again.
