Wiki Injections and Modifiers - bill for theraputic

I would like to ask for someones assistance with the following. I have been getting denials for the following billing examples:

99213
J3420
96372

The denial states that this is not billable without the appropriate modifier. Can someone guide me as to how you are supposed to bill for theraputic, diaognostic injections?

You just need a modifier 25 on the E/M service
