Wiki Injection Codes

Can anyone tell me when the medicare injection codes G0008 G0009 and G0010 were deleted. I didn't see them on the list of 2014 deleted HPCPS codes but they are no longer being paid. Any answers would be helpful.

I am getting paid for those 3 codes. Could it be an LCD-type of thing, just based on where you are?
