We have a patient that came into our facility the provider stated he is here today for follow up on FB was seen in Kansas at a eye clinic and had a foreign body removed. I am thinking his visit with us would be subsequent but the provider is saying initial.
 
It states in the guidelines if the patient is receiving "active treatment" with examples of active treatment being emergency room encounter, surgery, and evaluation and TREATMENT by a new provider.That would be the question was there active treatment or just follow up? If the patient came in just for follow up to have the eye looked at to make sure it has healed and there is nothing else wrong with the eye that requires treatment it would be a subsequent encounter even though the patient is new to the provider.

The guidelines were revised in Novmber of 2014. It no longer states as an example of initial, evaluation and treatment by new provider. It now state continuing treatment by same or different provider.
Also they clarified that initial is used for encounters for when the patient is receiving active treatment and not whether your provoder is seeing the patient for the first time.
So to answer the post regarding the follow up for the FB that visit is subsequent provided the FB was removed/treated prior to the encounter.
