Wiki Inclusive / Bundled / Global / Mutually Exclusive / medical necessity denials of CPT 88342 & 88360

We are receiving frequent denials for billing CPT 88342 and 88360 as "Procedure code incidental to primary procedure" or " bundled/ inclusive with 88360 " from BCBS, Aetna, Healthnet Ambetter, devoted health even after append modifier 59 with CPT 88342 as per NCCI. We have appealed the claims with pathology reports, but still not getting reimbursed. Any help would be great.
