I have a practice that uses A LOT of incident to visits - It's an Oncology practice. I want to make sure that if ANYTHING changes in the plan of care or new diagnosis the incident-to is lost. Where can I find this from an oficial source to confirm. Also, can it be easy as 'recommending' something as simple as an OTC medication for constipation? In oncology there are many side affects with treatment. Thanks.

In the incident to language it states that the physician must must have already exiled the patient for the same problem and has a written plan of care on the chart notes that the NPP is to use for the follow up visit. A new dx or a need to adjust the treatment will require a new plan of care and therefore it cannot be incident to
