If you have a NPP (Provider A) that is credentialed with an insurance company who sees a patient and establishes a care plan, can you bill a follow-up visit for a different, non-credentialed, mid-level (Provider B) incident-to the first NPP (Provider A)? Or can mid-levels only be billed incident-to physicians (MD/DO)?
