Wiki Incident to Billing with NP's and PA's

I am having conflicting issues with incident to billing. Back in 2015 NP's and PA's were Indirect billing for UHC (Commercial) and now some sources are saying that UHC (Commercial) is now direct billing for UHC. But when we send it to UHC, they pay as a Specialist and not a regular physician. I have been on the UHC website and all I see is for UHC Medicare Advantage.

When we bill UHC for incident-to we do the PA as rendering our MD as supervising and we use modifier SA and have not had any issue with getting this reimbursed.

Does anyone know if the SA modifier is appropriate to use for RHC?
