Wiki Incident to Billing for NP

We are starting to bill for an NP and need some guidance with incident to billing.
1. If the NP sees Dr. A's patient but Dr. B is the physician in the office, can we still bill under Dr. A?
2. If we bill under the physician incident to rather than under the NP, is the reimbursement 85% or 100%?

Any help is greatly appreciated! :)
 
Incident to is billed under the physician and paid at the physician rate. The physician must be immediately available to assist and in the office suite. You cannot bill under provider A if Provider A is not in the office. If Physician B is immediately available to assist and in the office suite the it can only be billed under Dr B
