Wiki Incident 2 billing

Can someopne please tell where to find information on incident 2 billing, I need to fine some info on when a Mid level is using a certain Dr. name to bill under, and that Dr. is vacation or not on the location how is this handled?? There is conflicting infomation here where I work.

There is an awful lot of information out there. IMO, you cannot bill under a physician's name if he/she is on vacation. If the visit meets incident to guidelines you bill under a provider name who is in the office that day and is acting as supervisor. If mid level is alone in the office, you must bill under the mid level's own name.
