Would I use 33216 for implantation of CS pace/sense lead via left axillary vein,
successful implantation of venoplasty of targeted CS branch.

33224 or 33225 for coronary sinus lead

The lead could not be placed at time of ICD implant. Would I still use 33224 or 33225? Thanks for your help.

Yes. Use 33224 when the generator is already present - same generator; use 33225 with a new generator.
