To recover the loss of revenue spent on disposables used in a surgical procedure, is it appropriate to have the vendor create an invoice for a "kit" instead of an itemized invoice so you can bill the carrier for the "kit"? Also, if you have trouble acquiring invoices from your vendors, is it also appropriate to create your own invoice if the vendor allows you to?
