Doctor did an exam and evaluation of permanent impairment disability rating, AMA guidelines, for a patient that had fractured ankle, chronic pain, and nonunion. Auto insurance is requesting copy of this report. Copies of previous medical records have already been furnished to Auto Insurance.

Can I use 99455 for this evaluation and do I have to use modifier 32?

Need to know if this coding is appropriate?
