If a physician provides immunization counseling, but a parent refuses the recommended immunizations for their child, is there an appropriate CPT code for this situation?

You can try DX V64.05 as the DX for 90465 and the CPT code for the vaccine and put a $0.00 charge for the vaccine and see if they pay. Make sure the counseling was documented in the chart.

For the ICD-9-CM codes, I would report V65.49 for the counseling and V64.05 for the vaccine not carried out because of caregiver refusal. Code V64.05 is a secondary code only. I would not report 90465. This is for vaccine administration and you did not administer the vaccine. If the visit was strictly for the vaccine and counseling, I would report a code from the preventative counseling series (99401-99404).
