Wiki ICD9 Codes Vs. Written diagnosis

Does anyone know if there is a policy or mandate that states, tha a provider is not allowed to use ICD9 codes instead of a written diagnosis? If you know the answer to this and have the documentation, kindly forward it to me medcodes1@gmail.com
