If my orthopedic surgeon sees a patient in the office for a fracture and schedules them for a repair in the OR I know the initial encounter will be coded as such, but when I code the surgery, is the diagnosis going to change to subsequent encounter?
I am so confused by all this already.

You code the initial seventh character until the injury is treated. Subsequent is for the visit subsequent to the treatment, not necessarily the subsequent encounter. SO for your scenario you would code the seventh character for initial for the ov as well as the surgery.
