My physician diagnosed a patient as having "Abnormal vaginal bleeding refractory to progesterone's." He changed her medications from Provera (progesterone) to desogen. I realize I have to code abnormal vaginal bleeding however, I can't find a code that indicates the progesterone was ineffective. I found codes that refer to when antibiotics are ineffective for an infections but nothing for this situation.

I agree on the vaginal bleeding as the primary diagnosis. I am unaware in ICD-9 of any code that represents an ineffective treatment protocol for any situation other than infection drug resistance. However there is a nonspecific V code that might be used in this circumstance, althought it's unlikely to impact payment. V49.89, other specified circumstance influencing health status.
