Coding HTN/CKD
Understanding the guidelines assume a cause/effect relationship would you need supporting documentation for both conditions to code? What kind of documentation would you need?

ICD-9 assumes a causal relationship between chronic kidney disease and HTN. So all you need is the physician to document both conditions and you will use a 403.xx code first and the CKD code second. This is stated in the coding guidelines

Thank you. I understand the cause/effect relationship but wouldn't there still need tobe supporting documentation of each condition, such as labs, meds, as required for any submission?

The physician needs to document HTN and CKD. As far as supporting test results, I would have to presume there is something that leads the physician to that dx.
