I work with Orthopedic surgeons and we often refer to our in house Physical therapists. Would the first visit with the physical therapist be seen as an initial? we do bill under one tax id. Thank you
 
No this was clarified in the 2015 guidelines. These are payient codes. Initial is used when this is the patients initial encounter for active treatment of the injury, it is not based on whether this is the patient first encounter with the provider. After a fracture has been reduced and traction or cast applied, then the encounter goes to subsequent it will remain subsequent during the healing phase, after the fx is healed and you send them to PT for the atrophy or gait instability for instance then it is sequela.
