We have a new urologist who is mixing his own IC cocktails. I am having trouble finding some of the hcpcs codes for the medicines he used. One is Neomycin and Polymyxin b Sulfates, NDC 00591219045, and the other is Lidocaine, NDC 00409427902. A nurse gave me J2450 and J2001, respectively, but these are not valid codes. I did finally find some info on the first med and it let me to the J3490, unclassified drug. If anyone can please help me with coding these meds, I would be so grateful. Thank you.
