I need help in coding this procedure:

Incision and Drainage with C/S of granuloma/wound right hand done as
outpatient surgery

DX: Granuloma
Chemical Reaction to Splinter

Within operative note Dr dictated
Subcutaneous with active bleeding from granuloma
No foreign body found
Skin under graunuloma was macerated
