How do we code for injections of Botox for hyperhydrosis in the feet and palms? I know to use the 64614 code but we did 160 injections because we treated both feet and palms in the same visit. We don't use the J-code because we get the Botox through the patients pharmacy. thanks

Check on series 64650-3. You're in the sympathetic nerves section, and you'll see the direction to report for hands or feet "64999."

I agree with the other poster. 64614 would not be the correct code in the procedure indicated. Consider 64650 which indicates both axillae and carefully read 64653 as it will guide you to the proper code for the feet ---> 64999
