I have a resident with CAD, HTN, and Acute on chronic renal failure... I just have a question on how to code the htn... would I use code 404.90 for hypertension with CAD with renal involvement? Or would I just use code 402.90 for hypertension with CAD?

It really depends on how the provider stated it in his note.
