When a patient has multiple problems and the document has two or three different HPI for each problem, do you add up the HPI or does it have to be 4 HPI per problem to meet a comprehensive HPI? Thanks for your help!
 
CC: Chest pain and nausea

The patient is a 50 year old female who developed sudden onset of chest pain (location), which began an hour ago (duration). The pain is described as stabbing (quality) and is rated 8 out of 10 (severity) in terms of intensity.

Our providers will separate everything out. They will state the problem and give HPI and then state another problem and give HPI. The problems are usually unrelated. What would you do in that case?
 
our RN Coder/Auditor said you just need 4 total, so you don't have to have 4 for each problem. does that make sense?
