Nursing home pt was sent to ER because unresponsive and was returned to the NH with dx of acute embolic CVA. Provider saw pt the next day and obtained history from chart and hospital records, however, ROS was unobtainable because pt unresponsive. Is that considered comprehensive history because provider documents status of patient and statement "ROS unobtainable, due to patient being unresponsive"?
