The patient presents for a sleep evaluation. The onset of the sleep problem has been gradual and has been occurring in a persistent pattern. The course has been constant. Daytime symptoms include daytime sleepiness, decreased energy, fatigue and dozing off during the day (when sedentary). The sleep problems have had an impact on the patients work but no impact on driving. The patient reports snoring, snoring every night and snoring that disturbs others. The patient reports mouth breathing during sleep and pauses observed by someone else during sleep but denies awakening with shortness of breath or awakening with a choking feeling. On workdays the patient wakes up at 9am and goes to bed at 1am. The patient wakes up to use the bathroom. During the first few minutes after awakening the patient feels very groggy.

I am able to pull: Timing, constant; Associated signs and symptoms, daytime sleepiness, decreased energy...;
Modifying factors, dozing off during day( when sedentary).

Can you count "groggy" for quality?
