Can I count chest pain as two elements in the HPI as CHEST for location and PAIN as asso.signs and symptoms?
And also it is acceptable if the Doctor is writing the notes for other CC's in the ROS section but not in the HPI SECTION (as an additional complaints with a PREVENTIVE VISIT) but he will put notations stating;Pls.refer to the ROS SECTION for other complaints ?We're using EMR.

Pain is the CC and chest is the location. You get only one HPI element. You can credit for additional HPI elements even if documented in ROS.
