Hello,
I work for a payer in the Fraud, Waste and Abuse department and currently auditing progress notes where I't clearly notes the name of the ancillary staff who documented the Chief complaint and the HPI. The DHHC CMS E/M service guide clearly indicates only the ROS and PFSH can be documented by ancillary. With that being said I believe we can recoup the money we paid since this would be a non compliant note. Would love to hear what everyones thoughts are on this matter.
