PT has a closed bilmalleolar fracture, an ORIF was preformed. Pt now has an allergy to the titanium and the hardware needs to be replaced. Would you use an unlisted code? A modifier on the ORIF code?

As a basic rule hardware that needs to be removed in order to perform a procedure is generally considered bundled. However, I have seen times when removing the hardware has been very time consuming and difficult. In those instances I feel adding -22 modifier is appropriate. If the same procedure needs to be repeated, look at modifiers -76 &-77.
