Wiki How to bill for discontinued Medication ?

Hi all, The drug Darzalex was prepared and then discarded. Patient had a health issue that prevented the patient from receiving treatment. Please clarify on how to document in the Medical Records as well as how to proceed with billing.

Was the infusion started and discontinued? If not, you cannot bill for it.

And I am assuming that you are a coder, so why would you be documenting in the medical record? That is the provider's job.
