Trying to find out how many days does a provide has to complete documentation after seeing a patient?

In the 2018 Medicare integrity manual it states the provider documentation is to be completed at the time of service. Late entries are acceptable as long as they are within 48 hours of the date of service. Anything beyond 48 is considered unacceptable as the provider cannot be expected to remember an individual encounter's specifics beyond 48 hours.
Thank you as we are having trouble getting Master Im or documents completed and sign off. This will help to let them know they do not have many days or week to get them completed. This is so helpful Thank you again so much.
