Pt comes in today with chief complaint of hyperhidrosis in the left residual limb. Pt has chronic infections and underwent left above the knee amputation. Subsequent to that she has been engaging in athletics and wearing prostheis. However, she has notice that when doing anything for a length of time she has hyperhidrosis in the stump. This causes a loss of suction with the prosthesis and large amt of wetness in her socket. this has been troublesome in that it does not allow her to perform all of her activities of daily living. Has no stump pain no breakdown or other issues . current meds include synthroid and ibuprofin.

I'm thinking this is an existing patient?

Although this is really ALL HPI, I think at best you'll be able to state you have 1-3 elements of HPI and a problem pertinent ROS (chornic infections).
