I'm hoping I'm in the right place...

I have a pt that was OBS on 12/25 and then on 12/26 she was inpatient and not OBS anymore. When it comes to the coding part, the doctor used 99219 on the first day which is OBS and then the second day they used 99232. Is this correct?? I was thinking you would have to use one of the 99221-99223 codes for the second day. The H & P was done on the 25th.

CMS provides guidance on how to code this situation. This is a direct quote from the following link and can be found on page 10: http://www.cms.gov/Regulations-and-Guidance/Guidance/Transmittals/downloads/R2282CP.pdf.

If the patient is admitted to inpatient status from hospital outpatient observation care subsequent to the date of initiation of observation services, the physician must bill an initial hospital visit for the services provided on that date.
