Wiki Hospital Discharge Summary

The hospital discharge summary has the admission diagnosis listed and the discharge diagnosis listed. On the admission list old MI is listed. It is not within the discharge list and nowhere in the chart can I find history of MI. Can we capture old MI from the admission diagnosis list?

Please advise.
