We are a specialty practice and one of our providers did a consult in the hospital for a patient and did two biopsies. Then went back a different day and did another biopsy. Now the patient is discharged and the doctor wants to see the patient in the office as a followup. We changed the place of service for the hospital visits.

Does the patient now become a new patient or do we bill as an established patient?

Established - the physician has provided face-to-face services to the patient within the last three years. I am assuming you are asking about the office visit - also, make sure you follow the global surgery rules for the biopsy. You may not be able to charge for the follow up visit if it is in the global period.
