I have a pt who was admitted to the hospital 4 days post op closed reduction percutaneous pin fixation of wrist. 7 days into her hospital stay for CHF the primary MD askes the Ortho who did the Sx to Consult since she should have had a post op visit with Ortho at this point. The Ortho does a detailed Consult and does a cast change. Is the only thing billable in this case the cast change? I understand the E/M is unbillable since she is post op and her admit is unrelated to her Sx from days prior. And she is not having any complications related to reduction. Right?
 
The ortho MD can only charge for the cast change if they physically performed it themselves since the patient is in an inpatient setting. You are correct, postop E&M visits are bundled by the performing surgeon.
