Does anyone know how to bill out home health plan of care? What is the correct CPT code to use and does anything else needs to be on the claim?

Help with medicare sequence coding of care plan oversight codes

Our provider recently got tagged by Medicare for not using the correct sequence for care plan oversight codes.

Currently we are billing G0180 for certification, G0179 for re-certification and G0181 for care plan oversight.

How should dates of service be reported for these codes and am I using the correct codes.

Please help!
