When do you begin using a "history of" or "late effects of" diagnosis for a patient who was initially seen for an acute injury, who months later, has an appointment for follow-up? Do you continue to use the acute injury code until the provider releases that patient from his care? Is there an indicated time frame for switching from an acute care dx code to a history of dx code?
