Wiki Hip revision ?

If a surgeon replaces the liner and the femoral head, would that be considered a hip
revision ( 27134 ) ?
He replaced the liner, replaced the cobalt chrome head, debrided osteolysis and a bone graft of the femur. He didn't replace the femoral stem, just the head.
I was not sure this was enough to be a revision both components. I am the only coder here so Any thoughts or suggestions would really be appreciated . Thank you

It's enough for the femoral component but what about the acetabulm? To capture 27134 you need BOTH components replaced..
